Chlamydia and gonorrhea are reportable to your local medical health officer as per Schedule A of the Public Health Act.
This can be done by completing the Chlamydia/Gonorrhea Clinician Reporting Form (previously known as the H208 form) and return by fax within five days to the IH Communicable Disease Unit at 250-549-6310.
