Public Interest Disclosure Act

Interior Health (IH) is committed to honesty, integrity and accountability in its operations, programs and services and promotes a culture of openness and transparency.

In accordance with British Columbia’s Public Interest Disclosure Act (PIDA), IH takes seriously all reports of wrongdoing and appreciates the courage of employees and health professionals who come forward to identify serious concerns and/or systemic wrongdoing that compromise the integrity of IH and the safety of employees, health professionals and clients/patients.

The information you provide on this form will be used to assess whether your report will be investigated. PIDA applies to all current and former IH Employees and Health Professionals, including the IH Board of Directors, and is comprised of the following kinds of serious and systemic wrongdoing:

  • A serious act or failure to act that, if proven, would constitute an offence under an enactment of British Columbia or Canada;
  • An act or failure to act that creates a substantial and specific danger to the life, health or safety of persons, or to the environment, other than a danger that is inherent in the performance of an employee's duties or functions;
  • A serious misuse of public funds or public assets;
  • Gross or systemic mismanagement; or
  • Knowingly directing or counselling a person to commit a wrongdoing described above.

If your concern does not meet the above criteria, the Public Interest Disclosure Act may not be the best way to report the wrongdoing in order to seek appropriate resolution. Matters that do not fall under PIDA may be dealt with through other established internal and external policies and reporting mechanisms. Please consider addressing the matter through Safe Reporting or contact your supervisor or union representative.

Disclosures under PIDA may be made anonymously; however, anonymous disclosures will only be considered where there is a reasonable basis to believe the discloser is an eligible current or former IH Employee, Health Professional, or Board Director. Anonymous disclosers should ensure they provide adequate particulars about their concern to allow us to assess whether the allegation(s) warrant investigation under PIDA. Anonymous disclosers should consider providing at least one form of contact information so that we can follow up to obtain more information, as needed. If we are unable to establish that a current or former IH Employee, Health Professional, or Board Director made the disclosure, the file will be closed.

The Public Interest Disclosure Act applies only to the following kinds of wrongdoing:  
  • A serious act or failure to act that, if proven, would constitute an offence under an enactment of British Columbia or Canada;

3. Please describe your concerns, keeping in mind how wrongdoing is defined, above. Explain how you learned about the wrongdoing and provide as much detail about the specific allegations as possible, including:
  • Where the wrongdoing happened or is likely to happen
  • Who committed or is about to commit wrongdoing (name, title and contact information)
  • When the wrongdoing occurred or is expected to occur
Please identify any applicable laws, Acts, Regulations or policies that may apply in relation to the wrongdoing
 

4. If you have reported the wrongdoing to your employer or any other person or organization, please provide details of who you reported to, when, their response and contact information.

 

5. If you know of any other complaint mechanism or organization that are addressing these allegations, please provide details.
 

6. If you consider the matter urgent, please explain why.
 

7. If you wish to include supporting documents related to the allegations of wrongdoing under PIDA, please email PIDA and attach any documents, records, correspondence, recordings or other evidence that you have in your possession  as well as any previous reports of the allegations you have made.


I have provided this information in good faith and on the reasonable belief that it could show a wrongdoing has occurred or is about to occur.

IH encourages good faith reporting of concerns, which means that you are protected from retaliation or reprisal if you honestly make a report. If you believe you experienced retaliation for making a report, you may provide that information on the e-Form as well as through our other contact options.

Your disclosure will be treated with the strictest confidence by our Designated Officers, who will review the information you provided while taking strict measures to keep your identity confidential. If additional information or clarification is required, the Designated Officer or their assigned representative will contact you directly.

FOIPPA Statement: This information is collected by the Interior Health Authority under s.26(a) and (c) of the Freedom of Information and Protection of Privacy Act and will be used to assess and investigate allegations of serious wrongdoing made under the Public Interest Disclosure Act.
