Our Substance Use Connections Teams support people who use drugs in both hospital and community settings. Clinicians who support this priority response provide the following services if you present with/or are at risk of overdose: - Short term follow up to support seamless linkages to community based addictions care - Referrals to Opioid Agonist Treatment - Liaison with Addiction Medicine Consultation Services - Assessment & treatment support - Safe discharge planning from hospital settings.
